The real cost of a missed call at your clinic (and how to stop losing patients)
How to calculate what missed calls cost your clinic, find out how many you miss, and stop the leak with callbacks, overflow answering and an AI receptionist.

The short answer
A missed call costs more than one appointment. It can lose a patient’s follow-ups, their family’s visits and the marketing spend that produced the call. Estimate your cost as: missed calls not returned × booking rate × value of a patient. Then stop the leak by tracking missed calls, calling back fast, and covering peak hours and after-hours with overflow answering or an AI receptionist.
Why clinics miss calls
Missed calls are rarely about effort. They happen when the front desk has more to do than it can.
- Peak OPD hours, when reception is checking in, billing and answering questions in person
- Two or more calls arriving at once on a single line
- Lunch breaks, shift changes and staff leave
- Evenings, Sundays and holidays, when patients have time to call
- Calls to a doctor’s personal number that never reach the desk
Patients rarely call twice
A patient looking for a doctor usually has several options on the same Google results page. If the first clinic does not answer, many simply call the next one. A callback an hour later often reaches someone who has already booked elsewhere.
How to calculate what missed calls cost you
The numbers in this example are an illustration. Replace them with your own.
- Count missed calls that were never returned in a typical week, for example 20
- Estimate how many would have booked; use your normal booking rate, for example 50%, which gives 10 patients
- Multiply by the value of a first visit, for example ₹500, which gives ₹5,000 a week
- Add follow-ups: if an average new patient returns twice more, the real figure is closer to ₹15,000 a week
- Add the marketing cost of those calls if they came from ads
Find out how many calls you really miss
Most clinics underestimate missed calls because nobody sees them. Check the call log on your phone system or exchange for missed and abandoned calls by hour of day. If you do not have one, a call-tracking number or a few test calls at peak hours will show the pattern quickly. Look for the hours with the most misses, not just the daily total.
Ways to stop missing calls, compared
Most clinics combine two or three of these.
| Fix | What it covers | Limits |
|---|---|---|
| Callback rule | Every missed call returned within a set time | Patients may have booked elsewhere by then |
| Second line or extra staff | More calls answered during staffed hours | Adds salary; nothing after hours |
| IVR menu | Routes callers to the right desk | Does not book; long menus frustrate callers |
| Outsourced call centre | Overflow and after-hours calls | Usually cannot see your live schedule |
| AI receptionist | Answers every call instantly, takes booking requests and hands over to staff | Clinical and urgent matters must still go to people |
Cover evenings, weekends and holidays
Many patients call when they finish work. If the clinic phone goes unanswered after hours, those enquiries go to whoever does answer. At minimum, use a message that tells callers how to book and when they will hear back. Better, let an AI receptionist answer routine questions and take booking requests, with urgent concerns directed to emergency services or the on-call process your clinic already uses.
What a good callback sounds like
Call back quickly, introduce the clinic, mention that you missed their call and offer a specific time rather than asking what they wanted. Example: “Hello, this is the clinic returning your call. We have a slot with the doctor at 6:30 this evening or 10 tomorrow morning. Would either suit you?”
Frequently asked questions
How many missed calls is normal for a clinic?
There is no reliable benchmark, and it varies with staffing and call volume. Measure your own missed calls by hour, then aim to return every one the same day and reduce misses at peak hours first.
Should I call back every missed call?
Yes. Return every missed call as soon as possible. A fast callback with a specific available slot recovers many patients who would otherwise book elsewhere.
Can an AI receptionist answer clinic calls?
Yes. It can answer routine questions, offer available slots and take booking requests at any hour, and pass clinical, urgent or complex calls to staff with context.
How do I track missed calls at my clinic?
Use the call log on your phone system or exchange, a call-tracking number, or reception software that records every call and whether it was answered or returned.
